When regenerative medicine becomes part of your healthcare, there’s a surprisingly important question you may have to answer: whose cells do you want to use?

That question was at the center of the POSEIDON trial, a landmark randomized study published in JAMA that directly compared donor-derived mesenchymal stem cells with stem cells collected from the patients themselves.
The study enrolled people with ischemic cardiomyopathy — heart damage caused by reduced blood flow, typically after a heart attack.
Researchers divided participants into two groups. One received allogeneic MSCs, meaning cells from a healthy donor. The other received autologous MSCs, meaning the patient’s own cells.
Each patient received one of three doses — 20 million, 100 million, or 200 million cells — injected directly into damaged areas of the heart.
The first thing researchers wanted to evaluate was safety. And on that front, both approaches performed well.
Donor cells did not trigger significant donor-specific immune reactions, an important finding that helped establish allogeneic MSCs as a viable area of research. Both autologous and allogeneic cells were also associated with reductions in infarct size and improvements in measures of cardiac remodeling.
But then the researchers looked at how patients were actually feeling and functioning.
And this is where the patients’ own cells stood out.
Patients who received autologous MSCs significantly improved the distance they could walk in six minutes.
At 12 months, that group was walking an average of about 66 meters farther than they had before treatment.
They also showed significant improvements on the Minnesota Living with Heart Failure Questionnaire, a measure of how heart failure affects everyday quality of life.
Those same statistically significant improvements were not seen in the donor-cell group.
